Patient
Confidentiality
If
you wish to speak to a receptionist or other members of staff
privately please do not hesitate to ask at reception. If you
prefer to leave a written message ask for writing materials
so that you can write your request, all doctor/patient cobnsultations are confidential.
Access
to Patients’ Records
Patients
wishing to have access to their medical records should give
notice in writing of their requirements. A fee for any photocopying
will be made plus administration costs. The surgery is required
to meet the requirements of the patient regarding access within
40 days. Requests for information required by a third party,
at a patient’s request, should be accompanied by a declaration
signed by the patient to release such information.
Car
Parking (at owner’s risk)
There
is limited space available on the car park at the front of the
surgery for patients. The practice cannot accept responsibility
for loss of or damage to vehicles. The spaces designated for
Doctors should not be used as there are times when the doctors
need to leave the surgery urgently and any obstructions can
result in valuable time being wasted.
The
gates at the side of the surgery should not be blocked as they
provide access to the garages of private premises at the rear
of the surgery. You are also requested to park considerately
on Holly Lane and not obstruct the drive-ways of local residents.
Actions
to be taken against violent or abusive patients
Our
practice clinicians and staff are here to help. They deserve
to be treated with respect. Poplars Surgery has a ‘Zero
Tolerance' policy of not accepting abusive or violent behaviour
from any members of the public. Any patients who do not abide
by this policy will be removed from the practice list and will
be written to advising them of the reason for the removal.
